The American Urological Association developed a BPH Symptom Index Questionnaire that gives you an idea if there is a prostate problem. The questionaire calculates the AUA BPH Symptom Index Score that is often used by urologists to determine the severity of prostate problems.

Less than 1 time in 5 = 1
Less than half the time = 2
About half the time = 3
More than half the time = 4
Almost always = 5
A total score of 0-7 is mild BPH, 8-19 is moderate BPH, and over 20 is considered severe BPH.
